Dr. Jiang Houren was invited to Taichung Veterans General Hospital to give a special lecture on "Adjuvant procedures and bone grafting technique of Orthognathic surgery."
Special thanks to Director Cheng Zhisheng and Dr. Tang Yuhui of the Department of Oral and Maxillofacial Surgery for their invitation and organization, which made this academic exchange full of depth and enlightenment.

Bone grafting in orthognathic surgery not only fills bone defects and enhances postoperative bony support, but also improves overall stability. Adjuvant procedures, such as bone plate fixation, can also help patients achieve more ideal functional and aesthetic results.
These technologies are particularly valuable for patients with facial bone defects, severe malocclusion, and craniofacial defects.

Snoring treatment success stories

Sleep apnea: Retraction of the mandible compresses the airway

Discuss the treatment case of a patient with severe sleep apnea. The patient's AHI index is as high as 84.5, and he has a typical face with a receding mandible. After airway lifting treatment through orthognathic surgery, the AHI index was significantly reduced to 5.6, and the minimum blood oxygen concentration was increased from 50% to 90%. The surgery not only effectively improves the airway space, but also makes the patient's jaw proportions more harmonious. The case details the surgical plan, postoperative changes and recovery progress, providing treatment reference for patients with similar problems.

Low AHI but accompanied by severe symptoms? MMA orthognathic surgery combined with invisible braces to reconstruct the airway, bite, and vertical proportions.

A 26-year-old male with an AHI of only 7.9 suffered from severe snoring, daytime sleepiness, and lethargy. 3D CT scans revealed a receding jaw, short face, and narrow airway. Through a combination of invisible braces and MMA maxillofacial repositioning surgery, airway space, deep bite, occlusion, and lower facial proportions were simultaneously improved. This case demonstrates that mild sleep apnea cannot be diagnosed solely based on AHI.

29-year-old man undergoes orthognathic surgery to completely cure apnea

A 29-year-old male patient was troubled by severe sleep apnea (AHI=39.6) and was unable to adapt to positive pressure respirator treatment. Through 3D customized orthognathic surgery, counterclockwise rotation technology is used to rotate the upper and lower jaw bones forward and perform bilateral turbinoplasty. The postoperative results were significant: AHI dropped to 4.7 (improved by 88.13%), minimum blood oxygenation increased from 79% to 94%, and respiratory tract volume increased by 76.6%. This case demonstrates the effectiveness of orthognathic surgery in the treatment of sleep apnea.

This article takes you inside the operating room, from pre- and post-operative images and 3D digital surgical planning to CT scans, to understand the relationship between a receding chin and airway narrowing, and how MMA orthognathic surgery increases airway space by moving the jawbone forward.

Treatment of sleep apnea in a 28-year-old man

A 28-year-old man sought treatment for severe sleep apnea. The preoperative AHI index was 45 and the lowest blood oxygen concentration was 76%. After diagnosis, it was found that the upper and lower jaw bones were underdeveloped, resulting in airway compression. Through 3D customized orthognathic surgery, counterclockwise rotation and forward movement of both jaws were performed. After the operation, the AHI index dropped significantly to 0.4, and the lowest blood oxygen concentration increased to 92%. The patient's symptoms such as snoring, light sleep and daytime sleepiness have been significantly improved, the respiratory space has been effectively improved, and the treatment effect is excellent.

Orthognathic surgery was originally designed to improve the bite and appearance of the patient. However, in the field of treating obstructive sleep apnea, orthognathic surgery (maxillo-mandibular advancement/MMA) has become a means to tighten the collapsed (relaxed) muscles rather than an end. Improvement in appearance and bite has become an added value of maxillo-mandibular advancement.
